An OCHD Registered Nurse
will follow infants with eligible conditions up to the age of 2
years. GOALS of follow-up services are:
- Promote optimal growth and development
- Teach the family care of a high risk infant
- Prevent complications and illness
- Decrease stress and provide support to parents
- Ensure early identification of potential problems and make referrals
as needed for treatment and evaluation
Eligible Children
High risk infant follow-up
may serve infants who have the following conditions, as determined
by the primary physician and/or the nurse case managers at OCHD:
- discharge from a neonatal intensive care unit
- serious congenital infection, congenital anomaly, or other serious
condition
- an endocrine, metabolic or immune disorder >a blood disorder
- birth weight less than 1,501 grams
- positive urine test for any drug or symptoms of drug withdrawal
- at high risk for medical complications for whatever the reason
Contact/Home Visits
The Public Health Nurse will make either a home visit or have a
face-to-face visit at our offices with each high risk infant as
closely as possible according to the following schedule:
- Birth
- 2 months
- 4 months
- 6 months
- 12 months
- 18 months
- 24 months

- Physical Assessment
- Nutritional/Diet evaluation and information
- Safety Suggestions for home environment
- Questions & Answers for parents/caregivers
- Referrals for other services if needed
